How will it help participants?

Allowing the patient to receive their medications without having to pick between purchasing their medicines or their mortgage, rent payment, utilities, or even food.

Who is it for?

Anyone who pays $10 or more per month for their prescribed medication(s).

How does it work?

CAGI has access to Prescription Assistance Programs offered by all nationally pharmaceutical manufacturers. You are invited to fill out an application for your prescribed medications to be given freely or at a very low price.

After the application is submitted to CAGI, Wonda Smith will communicate to you if the pharmaceutical manufacturer has an assistance program for your medication and has accepted your application.
1.) Fill out the form. The application form will be on the CAGI website, or CAGI will have printed application forms at the CAGI office located at 3266 N. Meridian St. Indianapolis, IN 46208.
2.) Submit the form to CAGI.
3.) CAGI will contact you back with any questions, notice of approval, or notice of unapproved from the pharmaceutical manufacturers
4.) If approved, a signature page is needed to be signed by the participating patient.
5.) A copy of the written prescription is needed.
6.) If approved, the medication will either be shipped to your home, pharmacy, clinic, or hospital. Never will your medications be shipped to CAGI.
