To become a STAR Program participant please download the application and list of required documents by clicking the button below and turn into our office. You can also apply at our office at 3266 N. Meridian St., Indianapolis, IN 46208, Suite 200 (8:30am – 5pm, Monday-Friday) or call 317-396-1800 and ask about the “STAR Program”.
